The clinic you consult with may have additional terms that govern your care relationship; where the two differ for matters of clinical care, the clinic’s terms control.

The information on these pages is for general educational purposes and is not medical advice. It does not replace a consultation with a licensed healthcare provider. Completing an intake does not create a provider-patient relationship and does not guarantee that any treatment will be prescribed. Treatment decisions are made solely by a licensed provider after reviewing your information.

Eligibility and accurate information

You must be of legal age and located in a jurisdiction where the clinic is able to offer care. You agree to provide accurate, complete information during intake. Providers rely on what you share to make safe clinical decisions, so inaccurate or incomplete information can affect your eligibility and your safety.

Payments and cancellations

Pricing, billing, refunds, and cancellation terms are set by the clinic and shown on the clinic’s storefront before you are charged. Please review your clinic’s pricing and refund details, which appear on its storefront, before completing a purchase.
